Let?s talk about prostate cancer. American men have a 16% chance of being diagnosed with it, though they have only a 3% chance of dying from it. Most prostate cancers grow very slowly, which means that you?re more likely to die WITH it than FROM it, according to Dr. David G. Williams.
The New York Times and the Washington Post have both reported that the PSA test is far from a definitive test for prostate cancer?in fact, it?s pretty useless. As I speak to prostate cancer patients, I am always struck by their fear of, and careful documentation of, widely swinging PSA numbers. And that?s how prostate cancer is diagnosed. We spend $3 billion annually testing PSA levels in men.
The PSA test measures how much prostate antigen a man has in his blood, which simply indicates inflammation level. However, any number of other causes, such as benign swelling (almost all men over 50 develop ?benign prostatic hyperplasia?), Ibuprofen and other drugs, or infections, can elevate the PSA even though it is unrelated to cancer. But very few doctors point this out to patients, nor do most seem to be aware of the controversy.
Dr. Thomas Stamey of Stanford University is one of the original PSA promoters. In 2004 he stated, ?PSA no longer has a relationship to prostate cancer. The PSA test is not relevant any more. You? might as well biopsy a man because he has blue eyes.? (Feel free to google Stamey and his comments and read many experts quoting him and discussing the PSA controversy.)
Stamey told Medscape Today?s Laurie Barclay, M.D.: ??About 20 years ago?we reported?there was a reasonable relationship between the blood PSA and the size of the?cancer. In fact, that relationship was approximately 50%, so it wasn?t even perfect 20 years ago when we first started using PSA to diagnose prostate cancer?in the last five years it?s now only 2%, so that?s negligible. That allows me to say that PSA today in the United States no longer has any relationship to the cancer except for 2% of men.? He states that a new cancer marker is needed.
The PSA test has been so ineffective in detecting prostate cancer, its inventor, Richard Ablin, has been speaking out against his own discovery for many years. Ablin is quoted in a March 2010 edition of The New York Times saying, ?The [PSA] test is hardly more effective than a coin toss. As I?ve been trying to make clear for many years now, PSA testing can?t detect prostate cancer?.The test?s popularity has led to a hugely expensive public health disaster.?
Biopsy or removal of the prostate not only causes incontinence and impotence, but also causes a dormant cancer to quickly spread. ?I wonder how many men have undergone surgery and radiation for no better reason than a PSA score, even though IF they even had a cancer cluster in their prostate, they may have had no symptoms for decades.
Dr. David G. Williams, whose newsletter ?Alternatives? I subscribed to for many years, feels that the PSA test is a travesty akin to mammograms for women, since it results in many ?false positives? and may contribute to causing cancer rather than diagnosing it.
I have been meaning to write about this subject for quite some time. I do so now because I made a Canadian friend at Hippocrates, a 64-y.o. horse rancher named Craig, who was there to deal with his prostate cancer. The good news includes that there?s no chemotherapy prescribed for prostate cancer, so he wasn?t recovering from that nightmare as many are. Plus Craig has given up his beer and animal-flesh diet, in favor of green juices, sprouts, and plants. He has lost over 30 lbs. and feels great.
But I told Craig that there?s a lot of underreported controversy about the PSA that he puts so much stock in. He had no idea. This post is dedicated to him and I?ll send him the link.
Since these experts have stated in the New York Times and Washington Post that research shows little or no correlation between PSA and prostate cancer, just last month both papers also reported that new research shows PSA screening has not saved lives.
In 2009, The New England Journal of Medicine published findings from two studies that screening didn?t reduce the death rate in the U.S. In Europe, it was slightly reduced, but 48 men would need to be treated to save one life. That means 47 men become incontinent, impotent, or both, to save one.

CARACAS, Venezuela (AP) ? President Hugo Chavez announced Tuesday that doctors in Cuba found a new lesion in the same place where he had a tumor removed last year and said he will require surgery.
"It is a small lesion of about 2 centimeters (less than 1 inch) in diameter, very clearly visible," Chavez told state television.
The announcement thrust Venezuelan politics into new uncertainty because the socialist leader is seeking re-election this year, hoping to extend his more than 13 years in power with a new six-year term.
He did not say when or where he would undergo the surgery, other than "in the coming days." He said he would meet with his inner circle to decide and expected to provide more details after a Wednesday Cabinet meeting.
Chavez, 57, said it should be less complicated than what he underwent in Cuba last June, when doctors removed a cancerous tumor from his pelvic region.
From July to September, Chavez received four rounds of chemotherapy, both in Cuba and in Venezuela, and he subsequently said tests showed he was cancer-free.
On Tuesday, Chavez denied rumors that the cancer had spread aggressively even as he said doctors do not know whether the new growth is malignant or not.
"I completely deny what's going around that I have metastasis in the liver or I don't know where, that the cancer has spread all over my body and that I'm already dying," he said.
He has never specified the cancer's exact nature or location, and opposition politicians and critics have repeatedly accused Chavez of a lack of transparency.
Analyst Cynthia Arnson of the Woodrow Wilson International Center in Washington said Tuesday's announcement seriously complicates Chavez's prospects for re-election on Oct. 7.
"It's now clear that Chavez's cancer is far from cured. Chavez's illness ? his ability to campaign as well as to govern ? is a major factor in the race. It erodes the aura of invincibility as well as inevitability that Chavez has always tried to create," she said.
The governing party will also be vexed as it lacks an alternative with Chavez's charisma and popular following, Arnson said. She predicted Tuesday's news will make "a tight race even tighter" against opposition candidate Henrique Capriles, a 39-year-old state governor.
Speaking from his home state of Barinas, Chavez said he decided to go public with the information about his need for surgery "because there are so many rumors, including rumors (the cancer) has been spreading."
Chavez, whose approval ratings have topped 50 percent in recent polls, had been out of public sight since Friday, and his government's handling of unconfirmed reports that he spent the weekend in Cuba for medical tests turned out to be ham-fisted. On Monday, repeated attempts by The Associated Press to confirm the reports went unanswered, and Communications Minister Andres Izarra vehemently denied them online.
"Regarding the rumors, dirty war from the gutter," Izarra tweeted.
Also Monday, an employee of the Venezuela Embassy in Havana said there was no indication that Chavez had gone to Cuba or planned to do so. The person did say that some family members of the president were in Cuba but had already left. The employee spoke on condition of anonymity, lacking authorization to discuss the matter publicly.
Security was noticeably tight Friday and Saturday at the Cimeq hospital in Havana, considered the country's best and believed to be where Chavez, Fidel Castro and other powerful people have sought treatment.
In recent weeks, Chavez has recovered the hair he shaved off during chemotherapy and he has appeared vigorous, albeit puffy around the face and neck. He had returned to a full schedule of activities including marathon television appearances. It was not immediately clear how his new surgery and recuperation from it might affect his routine and campaigning.
"I am in good physical shape to confront this new battle," Chavez said Tuesday.
But he also waxed on his own mortality:
"It's a life that I ask God to extend as long as he pleases only, if only, to be able to better serve each and every day the Venezuelan people."
Doctors consulted by The Associated Press said Chavez simply has not provided enough information to assess his prognosis.
Dr. Javier Cebrian, a colorectal specialist and chief surgeon at University Hospital in Caracas, said news that the lesion was in the very place the initial tumor was removed was not a good sign.
"A local recurrence is a bad symptom because it means the illness is growing again," he said.
"It's an ominous sign," said Dr. Michael Pishvaian, a Georgetown University oncologist. He said doctors often use the term lesion to refer to a new tumor, which appears to fit Chavez's description.
He said such a reappearance, particularly when a patient has undergone surgery then chemotherapy, suggests cancerous cells have resisted the treatments.
A leading Colombian oncologist, Dr. Carlos Castro, said conflicting information provided by the Venezuelan president himself made any assessment extremely difficult.
"He says he doesn't have metastasis. But a week ago he said he hadn't had a recurrence. And now he says that he needs to be operated on again," Castro told the AP in Bogota, Colombia.
Castro said Chavez could be expected to need radiation therapy if another tumor is removed and that typically means a minimum of 10 daily sessions, which means Chavez would need to name a temporary replacement while undergoing treatment.
Capriles' campaign coordinator, Armando Briquet, said he and his team wish Chavez "a complete recovery" and "a long life although we have always been critical about the lack of real information about the president's health."
"We don't know exactly the extent, or what he has, or where he has it," Briquet added.
Capriles claims Chavez has exploited his lengthy rule to balance the scales against a fair election, taking advantage of government money and slanted coverage in state media.
He is a strident critic of Chavez's expropriations of hundreds of businesses, apartment buildings and farms over the past decade.
The government's generous spending has made Chavez a hero to many of his supporters, which make up a large segment of Venezuela's poor.
Opponents say Chavez has done nothing to combat Venezuela's rampant violent crime and blame him for 26 percent inflation. His opponents also criticize the former paratroop commander for his strident anti-U.S. rhetoric and defense of Iran and its nuclear program.
The U.S. Embassy in Caracas has been without an ambassador since July 2010.

The international evidence suggests that while the prevalence of smoking per se has decreased, social smoking - smoking intermittently or only in given situations - has increased among young adults, say the authors.
The authors carried out in-depth interviews in 2011 with 13 "social smokers," aged between 19 and 25, who were recruited through the online social network Facebook and via posters in cafes, supermarkets, and on community noticeboards.
Analysis of the transcripts showed that social smokers often had conflicted identities. They found it very difficult to reconcile their stated identity as non-smokers, who smoke.
They managed this conflict by limiting where and when they smoked and by sharply differentiating themselves from "addicted" smokers to whom, by and large, they felt superior, using several strategies.
These included claiming never to smoke alone; asserting that they controlled when, where, and how much they smoked; and defining their smoking as "a temporary phase."
They also rationalised their behaviour by saying that it only occurred when they had been drinking, describing smoking and drinking as going "hand in hand." Some said that alcohol prompted cravings for a cigarette, which they wouldn't otherwise experience.
But alcohol also enabled them to absolve themselves of any responsibility for their actions, which they inevitably subsequently regretted.
Drinking therefore enabled them to "binge smoke," while also distancing them from this behaviour, so helping to maintain their "non-smoker" persona.
One respondent commented: "Some nights I can smoke 14/15 ciggies or a pack while I'm drinking but I can never do that without alcohol."
When asked for their views on mandating smoke free areas outside bars, which could help decouple smoking and drinking, all but one participant strongly backed this proposal, and indicated that it would help them cut down or stop smoking.
"Introducing smoke-free outdoors bars could reduce social smoking by removing cues that stimulate this behaviour and changing the environment that facilitates it," suggest the authors.
"Such a policy would eliminate the current intersection between smoke-free and smoking spaces and create a physical barrier that, for some, would make accessing the smoking zone too difficult," they conclude.

When asked for their views on mandating smoke free areas outside bars, which could help decouple smoking and drinking, all but one participant strongly backed this proposal, and indicated that it would help them cut down or stop smoking.
"Introducing smoke-free outdoors bars could reduce social smoking by removing cues that stimulate this behaviour and changing the environment that facilitates it," suggest the authors.
"Such a policy would eliminate the current intersection between smoke-free and smoking spaces and create a physical barrier that, for some, would make accessing the smoking zone too difficult," they conclude.

So, I went ahead and bought the do it at home package, and I am going to play with it in the coming a few days.
However,?last year I made quite good numbers, most of my income came from my design business and affiliate marketing.
By the way you can check a few of my income?reports?that I published before on the blog.
PayPal sent me a 1099-K of $56,990 gross amount of merchant card/third party network payments, and of course that?s not all, I had a few other 1099-K forms sent by some affiliate networks and one by Google AdSense, but PayPal is my biggest number.
This means I have some nice work to do this year to file my income Tax report!

Deal trims maximum jobless benefits to 73 weeks

WASHINGTON (AP) ? Unemployed workers would no longer be able to claim 99 weeks of benefit checks by this summer under a deal being worked out in Congress.
Under this week's compromise for extending a Social Security tax cut through the rest of 2012, federal unemployment benefits for people who have been out of work more than six months are being scaled back.
If Congress passes the bill and President Barack Obama signs it into law, the current maximum 99 weeks of benefits will gradually fall to 73 weeks by September. For people in all but about a dozen of the highest unemployment states, the benefits will be cut off after 63 weeks.
Democrats had hoped to keep the number as close as possible to 99 weeks, arguing that the benefits are critical for those struggling to make ends meet and provide a boost to the economy. Republicans wanted to reduce the maximum time span for benefits to 59 weeks, saying too many people don't seriously look for a job until the government checks quit coming.
The agreement would let both parties claim victory: Democrats can say they preserved the program for another year, while Republicans may claim they won major concessions by scaling back the program.
About 43 percent of the nation's nearly 13 million unemployed have been without work for more than six months, double the rate of any other economic downturn since the Great Depression. If Congress had not reached a deal to reauthorize the program, about a million people would have lost benefits next month.
"It's far from perfect, but it seems to be a responsible approach to the current problem of long-term unemployment," said George Wentworth, an attorney with the National Employment Law Project, a worker advocacy group that supports long-term benefits.
It was not immediately clear how many people might lose out on benefits later this year under the new plan. Currently, 22 states are eligible for 93-plus weeks of unemployment insurance; just 18 get the full maximum of 99 weeks. The average unemployed worker receives less than $300 a week in benefits.
Wentworth said the gradual decrease would help cushion the blow for those relying on the benefits.
The plan would extend the current 99-week maximum through May for states with the highest unemployment rates. Benefits would drop to 79 weeks in June and to 73 weeks in September. Unless Congress extends the federal benefits again, people losing their jobs after July 1 will get only 26 weeks.
"This agreement is a step in the right direction," said James Sherk, a policy analyst at the conservative Heritage Foundation. "However, two years of benefits was excessive when passed, and a year-and-a-half of benefits in an improving labor market is still excessive."
The program was already starting to wind down anyway. Under the current formula, the maximum coverage period would have fallen to 79 weeks in October.
The job market has been steadily improving, and fewer people are filing for the benefits. On Thursday, the Labor Department reported that the number of people seeking unemployment benefits last week was the lowest it's been in four years.
But Heidi Shierholz, a labor economist with the liberal Economic Policy Institute, said it's still too soon to cut back on benefits.
"Even if we continue growing at the current rate, given the depths of the job deficit that we're in, we're not going to be at a place by May where job opportunities are plentiful," she said. "Layoffs are back to pre-recession levels, but hiring is still extremely slow."
The new law will allow states to make benefit applicants take drug tests if they lost their job because they failed a drug test or are applying for a job that requires one. Republicans dropped an effort to let states require all applicants to take a drug test, or require applicants without a high school diplomas to pursue a GED certificate.
